In order to apply for an entry visa a completed Visa Application Form must be submitted either in Portuguese, or English. Each copy of the Application Form must bear a recent passport photo with a light background measuring 5 x 7 cm at most.
The following number of Application Form is to be submitted for each type of visa:
Tourist Visa (VITUR): one Application Form
Transit Visa (VITRAN): one Application Form
Diplomatic Visa (VIDIP): Verbal Note and one Application Form
Official Visa (VISOF): Verbal Note and one Application Form
Courtesy Visa (VICOR): Verbal Note and one Application Form
Temporary Visas (VITEM-I, VITEM-III, VITEM-IV, VITEM-V, VITEM-VI e VITEM-VII ): two Application Forms
Temporary Visa (VITEM-II): one Application Form
Permanent Visa (VIPER): two Application Forms
When two forms are required, the first form will be archived in the Consular Section and the second one, with the stamped photo, will be delivered to the applicant so that he can register with the Maritime, Aerial and Borders Police Division within 30 (thirty) days of his or her arrival in Brazil.
Where Temporary and Permanent Visas are concerned, the following instructions are to be rigorously adhered to when submitting the Application Form:

all spaces are to be filled out in either black or blue ink (block letters) or typed;

part of the photo and of the Form must be stamped by the Consular Section;

the length of stay in Brazil must be mentioned (in days or years);

must be stamped by the official responsible for visas; and

in the case of Permanent Visas, mention to the legal reference is requested in the space reserved for “Observations”.
